Examples
Here is sample language you could use in your syllabus from the Common Syllabus Statement Hub:
HCC is committed to creating a learning environment that meets the needs of its diverse student body. Any student with a disability who requires accommodations should contact the Office of Services for Students with Disabilities (OSSD). The OSSD staff works with students and instructors to identify reasonable accommodations and academic adjustments. If you experience any barriers to learning in your courses, contact the OSSD Coordinator on your campus for assistance.
We highly encourage you to submit your accommodation requests within the first two weeks of the semester because the accommodations are not applied retroactively. Register with the OSSD as soon as possible to determine accommodations, and an Academic Adjustment Memorandum will be emailed to the instructor for implementation.
Contact the Office of Services for Students with Disabilities (OSSD) for more information.
